You should complete this process only if you are an existing TALI Member and you wish to renew your Membership for 2008. Please refer to the Membership Renewal Statement you received from us when filling out this form, and please be sure to enter new and/or updated information below. Please remember that the information you provide on your Renewal Statement is the information used to compile the TALI Membership Directory. If you provide erroneous information to us, that is what will be published in the Membership Directory.

Full payment for your renewal must accompany this form or your renewal cannot be processed. All online payments are processed through PayPal. If you prefer not to use PayPal, you may fax your renewal form to us, together with your credit card information at 877-445-TALI (toll-free). Please include your full name as it appears on the credit card, the card number, the expiration date, the 3 or 4 digit security code, the address where you receive statements from the issuing bank, the amount which you are authorizing TALI to charge to your card, and please be sure to sign your fax.

Please note that ALL fields are REQUIRED except Birthdate, Company Name and Web Site Address. If you do not have information to provide in a given field (e.g., you don't have a fax number), please put "n/a" in the box. If information in a particular field has not changed from last year, you may put "n/c" in the box. Please be sure to provide your full name.
